Based roughly off the old goldenrod forms, spacing required it to be 3 pages instead of the printed front-back 2 pages. First page has Stats and Experience, second has Equipment, third has Languages and Notes. Unscripted, fill-in-the-blanks form with place for picture.

Updated this on 1/22 - Had a couple fields that were too small, changed a couple numeric fields so you could use % signs, and redid the modifiers section under AC (and added a checksum calculation).
